New Affinity Healthcare Office Opens

We are pleased to announce the opening of our new medical office building located at 1450 Busch Parkway, Buffalo Grove, IL  60089, on Monday, August 3, 2009.

The 1300 Busch Parkway,  Buffalo Grove, and

201 E. Strong Ave., Wheeling, primary care practices will move to this location on Saturday, August 1st and Sunday, August 2nd..

Note to patients of 1300 Busch Pkwy,  Buffalo Grove and 201 E. Strong, Wheeling offices:  the phone number to reach your Affinity Healthcare physician will remain the same at this new location.
